HOW TO ENROLL
We are so pleased that you have chosen CVCS for your child's education. CVCS is unique! We are the only public education option in the city of Chicago that seeks to educate children through a strong child-parent-teacher partnership through a blended education model. Enrollment is limited to just 680 students. All applications will be placed on the CVCS waiting list and will be offered seats based on grade-level applications and availability. CVCS collected initial applications for the lottery application, which would have taken place on April 12, 2019. Due to seat availability, seat offers have gone out and we did not require a formal lottery.

Students who are awarded a seat offer will have to become fully compliant before approval.
Eligibility Requirements include:
· Currently reside within the Chicago city limits
· For kindergartners, students must be five years old by September 1, 2019 in order to enroll.
ACCEPTANCE & FINAL ENROLLMENT
Upon acceptance to CVCS, you will be asked to provide the following documentation so that we can complete the enrollment process. You will receive further communication regarding this information, but please be sure to have the following information/documentation ready to submit for completion of enrollment:
· Birth Certificate
· Primary Proof of Residency: Driver’s License or State ID
· Secondary Proof of Residency: A document such as a utility bill with your address on it)
· Physical Form
· Immunization Records
· Most Recent Report Card (Elementary and Middle School)
· Unofficial/Official Transcript (High School)
· Copy of IEP or 504, as applicable
